Kaiserslautern is a city in southwest Germany.
30 Sept. The Cannoner Heinrich Jergens, at ____________ 12 here, received for outstanding bravery the Iron Cross 2nd. class.
That's as close as I can translate. One word totally evades me so far. Hopefully, someone can do better.

Hello, the missing words are "Am Stophelspfad 12" which is an adress (obviously the adress of Heinrich Jergens at these times) in Kaiserslautern.
