Nats, I think a German Red Cross decoration. The first class of the "ehrinzeichen des deutschen roten kreuzes ", in this case with diamonds and rarely awarded. The swas is obscured in the wreath no doubt to be politically correct. Can't comment on authenticity. Barry.

That is the German Social Welfare decoration 1st Class with Diamonds. In this case a not so good fake of this special class.
Here all Classes from all types of the German Social Welfare awards:

German Social Welfare Award (replaced the German Red Cross Awards on Apr.1939):

- Special Class (1.Class with Star; Note: No Grand Cross)
- 1.Class (Neckcross)
- 2.Class (Breastcross)
- 3.Class (on ribbon)
- 3.Class (Women's Cross on ribbon bow)
- 3.Class, special class with golden swords on ribbon (Instituted 30.jan 1942)
- Medal for German Social Welfare (on ribbon)
- Medal for German Social Welfare (Women's Medal on ribbon bow)
- Medal for German Social Welfare, special class with silver swords on ribbon (Instituted 30.jan 1942)

NOTE:
The 1st Class with Diamonds from Mrs. Antonescu (for example) is not a instituted class, it was more like a gift...

Nats, this is a social welfare medal, red cross would have a cross on the eagles chest. Like koj said, it's not a good one.
