These are buckram backed, and the person who made this (more than likely at home in this period..) has added a nice decorative touch to cover the plain buckram back and folded over ends of the base material.

My opinion is that the tab is for the rank of Sturmführer of SA-Gruppe Westfalen.
I'm sure Bob will confirm when he see the better pictures of the tab.

Early medical tabs of this rank were piped goldon/purple, and goldon pips.
