These are buckram backed, and the person who made this (more than likely at home in this period..) has added a nice decorative touch to cover the plain buckram back and folded over ends of the base material.

My opinion is that the tab is for the rank of Sturmf�hrer of SA-Gruppe Westfalen. I'm sure Bob will confirm when he see the better pictures of the tab.
Early medical tabs of this rank were piped goldon/purple, and goldon pips.
