Has anyone seen or heard of a SS collor tab with metal runes? The cuff title is supposed to be for a medical unit which I have never seen either.

Metal runes were found on some locally made insignia used by Baltic volunteers. There is good photographic evidence of their period use, primarily by Latvians. However, these do not resemble what would have been used by such soldiers. Also, even those which do look as if they could be original are impossible to "authenticate" without iron-clad provenance, because of their non-standard manufacture.

and they wouldn't have been used on with the 'twist piping' shown on those tabs, as that was doen away with early on in the war.

The did have a cufftitle like that, but the poor lines on the roman numerals makes me seriously doubt that one.

As bwanek1 mentions there is good [and plentiful] evidence of their period use, and it was mostly the Latvians using them..There are a few different styles but I remember none with twist piping..

Try a search on metal runes here. Some examples and/or photo evidence might still appear.

There's a good example in Andrew Steven's Waffen SS uniforms in colour photographs on pg. 39. Said they were mainly manufactured in the east.

I can only support the use of metal runes in Latvian units. It has also been found collar patches on the battlefield like the one you have...
